Bulk Pass Application

Legal Notice 

I am an officer or other duly designated representative of the organization, business, corporation, residential property (“entity”) with full authority to submit this application on behalf of the entity and commit the entity, if chosen by PRT, to participate in PRT’s Bulk Pass Pilot. 

By submitting this application, I attest that the entity for which I am applying on behalf of: 

1. Currently has a physical location within a half mile walkshed of PRT service to which employees or residents live, regularly work and/or consistently travel to and from; 
2. Agrees to the promote participation in the pilot, facilitate surveys to their employees or residents regarding the pilot, and fully cooperate with PRT in carrying out the pilot; 
3. Agrees to purchase passes for every employee or residential unit; and 
4. Will be responsible for distributing mobile ticketing passes to their employees or residents and maintaining the associated information, including maintaining records and responding to information gathering requests from PRT. 

I understand that submitting an application on behalf of the entity for the Bulk Pass Pilot Program will not be considered to affirmatively indicate the entity’s participation in this or any other similar program that may exist through PRT in the future.  Should such a program be implemented in the future, there will be a separate application process. I also acknowledge on behalf of the entity that the Bulk Pass Pilot Program may be modified, suspended and/or terminated in PRT’s sole and exclusive discretion.

I recognize that the current application period for the Bulk Pass Pilot Program is closed and my application will not be considered at this time.
